Since 2005, Cooperative Counseling Services was founded on the principles of offering comprehensive, compassionate care to residents of the State of New Jersey. Clients choose whether they access services through HIPAA compliant telehealth or in-person office setting. Treatment may begin in as few as 5 to 10 days after your intake form is processed. An online application and intake package must be completed prior to starting services.
Whether services are administered through telehealth or in-person, all clients receive the same high-quality care and positive clinical outcomes.
Cooperative Counseling Services accepts Medicaid, and participates in-network with most Aetna, Cigna, Horizon, and United Healthcare plans.

Please complete the New Client Information Package to access services. Once the complete package is submitted electronically, a team member will contact you to set up your first appointment (telehealth or in-person, your choice).
Please allow 2 business days for us to review your New Client Information Package. New clients are currently being scheduled for their 1st appointment between 5 and 10 after we process your completed intake package.

By completing the intake package, you are consenting to Cooperative Counseling Services review of the social, emotional, and medical history you provided. Mental health services are recommended based on the accuracy of the information you provide. Incomplete forms only delay the process.
*Appointments are not guaranteed to all applicants; services recommended will align with clinical presentation.
*Translation services are not available; however, bilingual (Spanish) therapy services are available.
OUR TELEHEALTH POLICY
Your health, safety and privacy are important to us. To ensure high quality telehealth services:
Sign into your appointment in a safe location (i.e., not while driving).
Ensure privacy and confidentiality by being alone in a room.
Parent or guardian must be available if telehealth patient is a minor.
You must be a resident of NJ with NJ issued insurance benefits.
