Two Ways to Become a New Client

Getting started should be easy. Reach out whichever way works best for you — by phone or online, on your schedule, intake appointments are available in person or virtually.

OPTION 1

Call Our Office

Speak with an Access Specialist who will collect your information and schedule your intake appointment with you, all in one call.

The call takes up to 15 minutes, so please have ready:

  • A little time set aside

  • A private, comfortable space

  • Your insurance information

Monday - Friday, 9:00 AM - 8:00 PM

OPTION 2

Register Online

Prefer to do it yourself? Complete the registration information on your own timeline — start now, finish later — and submit it to us anytime.

What happens next:

  • Submit whenever it's convenient— day or night

  • Our Access Center reviews your information

  • We'll get back to you as soon as possible

Available 24/7

New Client Package

Please complete the New Client Package. Once we receive it, our Access Center will review your information and call you to schedule an intake appointment, either in person or virtually.

By completing the package, you consent to Cooperative Counseling Services reviewing the social, emotional, and medical history you share. Please fill out every section as accurately as you can, since incomplete forms can delay the process.

To get help right away, or to schedule by phone, call our Access Center at 908-731-7099 x504.

Business hours: Monday–Friday, 9:00 AM – 8:00 PM

Please note:

  • An intake appointment does not guarantee ongoing therapeutic care. Based on your intake, we may recommend a different level of care, a psychiatric evaluation, or another service that better fits your needs. We will talk through any recommendations with you and decide next steps together. If a referral is the right fit, our team will help coordinate it.

  • Translation services are not available, but therapy in Spanish is offered.
